Amazing Grace (Gwen Bartley) is an amazing group of advocates to have on your side.  The knowledge and skills of this team are truly like nothing else I have experienced. I am always getting emails, ideas, webinars, group discussions and other community opportunities I would never have known about.  They cater to the issues that we are struggling with as a family.  I sent my son to their camp this summer, and he had an awesome day!  He also saw other kids struggling and to know you are not alone in the struggle… is everything for my son.  I am an Licensed Clinical Social Worker (LCSW) in NC and I work with young children and their families, however, I can’t say enough about why it is so important for every parent to have a program like this supporting you. Amazing Grace has been a godsend to our family. Gwen and her staff have been instrumental in assisting us through our difficult journey with our son who has ADHD, OCD, and ODD.  Camps like those provided by Amazing Grace are invaluable to families such as ours where options are few and far between. Parent/Youth support group that's offered, helps families to discuss their concerns with other families going through similar situations. Family Wellness Program-Individualized Family Service


              Available to families raising children (ages 0-21) with any condition that is affecting the Brain and it's function. 


​Individualized Family Service (IFS) is a type of case management that looks at the entire needs of the child and family.  We explore the following areas: Therapeutic, Medical, Educational, Recreation, Family Needs, Parent Self Care, Crisis Planning and Transition into Adulthood Planning.  Families are given materials and Action Plans to address all of the concerns with resolutions. 
Included in this Service:

  • Discovery, communication and Referrals with APPROPRIATE Mental Health and Medical Providers.
  • Educational Needs IEP/504/BIP and Homebound Meetings- We do require a 30 day notice for these meetings, so that we can review concerns and have a pre-meeting plan for best outcomes.
  • Recreation and community resources that will be a good fit for all members of your family.
  • Support and tools for families to function successfully, despite the challenges associated with Brain Disorders.
  • Crisis planning that is realistic and reduces the stress and trauma.
  • Complex conditions that require specialized treatments; including out of home placements, emergency Medicaid applications and long term care.
  • Transition into Adulthood starts at age 14 and continues until age 21. Direction and referrals with respect to what is important for your family.

Cost of Service: 

Consultation $35.00 This is a 1-2 hour meeting with family and navigator that looks at all aspects of family needs. A Guidebook and Action Plan is provided to family. Parents/caregivers can decide if they would like to manage Action Plan on their own, or continue with the Individualized Family Service and having a Navigator to assist with managing the Action Plan.


Individualized Family Service $25.00 per hour  Navigators manage the majority of the Action Plan, which typically requires 2-5 hours per month of service. This includes aligning of services, referrals, Triple P Parenting Program, communication and attendance of all educational concerns/meetings, locating community resources for recreation, support and opportunities. For complex conditions, we work directly with local and state resources to find the best services to help your child and family find stability. Transition to adulthood takes a comprehensive look at the short and long term needs of the child and family; aligning you with the appropriate adult services, agencies and supports needed for a successful and sustainable adulthood.


Our goal is to keep this service affordable for all families; we offer flexible payments, sliding scale discounts and some Pro-Bono cases to qualifying families.
